AI Found Your "Anemia" – And Nobody Treated It

The Blue Cross Blue Shield Association studied three years of claims covering 62 million Americans and found something stunning: hospitals using these tools showed a sharp spike in anemia diagnoses – but the treatments doctors actually administer for anemia didn't budge.
"If patients are truly sicker, we'd expect to see more treatment," said Luke Chalker, BCBSA's senior vice president of product and data science. "For example, we're seeing significantly more anemia diagnoses at these hospitals without a corresponding increase in transfusions. The disconnect between diagnoses and treatment suggests that AI is identifying more billable conditions, not sicker patients."

The AI flagged the condition.
The doctors didn't treat it.
The hospital billed for it anyway.
According to Blue Health Intelligence, BCBSA's data analytics arm, coding intensity drove roughly 20% of the rise in per-patient inpatient costs between 2023 and 2024.
Those secondary diagnoses alone added $653 million to Blue Cross plan costs over two years – with hospitals collecting an average of $11,800 extra per complex case.

DeSantis nailed it Monday: "Both Big Hospitals and Big Insurance are using AI tools to either seek more reimbursements or to deny more claims. The result: AI providers make a lot of money, individuals pay higher premiums and have more denials, and nobody gets healthier."
The American Hospital Association confirmed that commercial claim denials jumped 20% between 2022 and 2023 alone.
Medicare Advantage denials – the plans serving seniors – jumped 56% in that same window.
Hospitals now spend $19.7 billion a year just appealing those denials.
That cost doesn't disappear. It lands on your bill.
Meanwhile UnitedHealth Group – the same company whose AI denial practices made its CEO a national symbol of everything wrong with American healthcare – announced plans to spend $3 billion more embedding AI deeper into its claims process.

DeSantis already moved earlier this year to ban AI as the sole determinant for denying insurance claims in Florida.
He's also pushing transparency laws that force insurers and hospitals to disclose what these tools are actually doing inside their billing systems.
Six million Floridians carry Florida Blue coverage – making this a direct pocketbook fight for roughly one in four people in the state.

DeSantis is also pushing a shared savings model where patients who choose lower-cost options keep a portion of the savings themselves.
That's the opposite of the current system – which rewards hospitals for finding your "anemia" and rewards insurers for denying your hip replacement, while billing you more for both.
The UnitedHealthcare CEO's death sparked a national conversation about why Americans hate their insurance companies.
The Blue Cross report just handed that conversation a paper trail.
DeSantis is one of the only elected officials in the country treating it like the emergency it is.
